Sprung hose clip failure

2008 Astra H 1.6 petrol manual
Yesterday I was called out by my daughter to Leigh Delamere services M4. She had been returning to London after her Christmas break, and had lost all her coolant when the hose at the bottom of the coolant header tank had come off. There was no sign of the hose clip. I replaced the clip and coolant and sent her on her way.

That car has performed faultlessly for 3 months, losing no coolant. Previous day it went to South Wales and back. I can only presume the sprung clip had fractured, something I have never known before. Any comments?

Old rusty clip... Or perhaps a previous shaven ape has overtightened it after some work and she's gone over a bump.

I've seen a couple, but usually after I have been the shaven ape in question.

That's just one of my reasons for never refitting the damn things on my own cars. Good quality, correctly sized, worm-drive hose clamp every time.

Is completely and utterly the wrong thing to do, as well you know ;)
