iirc Challenger was human error in demanding a launch when it was too cold for the rubber seals to work?

I recall Challenger being that plus when investigated there was an underlying design issue with the seal system to.

I do recall what Jim says now (as well as Mark), however I do recall someone saying back then that even though the seals were flawed they would have held up if it hadn't been so cold when trying to launch.
At least on this occasion there was no loss of life, and maybe someone somewhere will realise you need to triple check everything when playing with rockets.
