Sorry if I have missed something here but should that not be suspension or chassis failure rather than brake failure ?  :-\

The whole issue starts from bad brake bias front-rear... Under normal braking you shouldn't be able to lock the rear wheels before the front ;)

Bearing in mind that the info says 15tonnes @ 60mph  ::)
