Or incorrectly fitted, ie torqued up with car still jacked up...

Hang on... they were very carefully torqued up, and angle tightened, with the wheels on the ground.

Anyway one of the bushes that has failed was a vertical one.....

The quality didn't seem great, but they looked OK.

Sorry, not specifically talking in your case  :-[, just in general, if not torqued up with car on ground, this can cause premature bush failure :y
