Not sure about the codes, but that actuator will be in the right hand front footwell. Remove the trim from beneath the glovebox, and you should just about see it above/behind the end of the centre console. Arm up is cold, arm down is hot.

The rod connecting the motor(actuator) to the valve in the matrix can pop off, symptom being that the temp on one side won't adjust. :y
