Head gasket

I have some pics here from today...What caused this? Head gaskets were done two years ago.

My gut reaction is the head/block surfaces not perfectly prepared/flat, although both look to be corroded.

Very early 1998 3.0l seemed to suffer issues whereby coolant could leak at back of no6 - my own 3.0l suffered this twice in my ownership.  Does your's fit into this?

Left and right head cilynders closest to firewall were leaking coolant into combustion chamber...But after only two years after heads were done?
