There are mistakes, and there are real mistakes with life or death consequences

It seems in this case as though the agency nurse had not received any training to deal with such an incident, but she decided to touch the machine.............!!!!!

The poor patient suspected his care was going to be substandard and installed CCTV.

What do we expect to happen?

I wondered how many times a similar thing had happened but not caught on CCTV. 100 times a year? Not good.

Yes, I think that's the theory behind it Chris but like the LEAN manufacturing post you commented on over on Richard's thread, there is quite a difference between theory and practice.  I wouldn't be at all surprised that employing agency staff actually cost more in the long run.

In this case it led to an unecessary tragedy - the agency staff member who allocated the job to this untrained 'nurse' (given the type of equipment involved) should have their arses kicked out into the gutter.

I think if you are staff then you are limited in the number of hours you can work in a day.  If you are on two agencies however....
