Councils/ highway repair gangs who don't bother to remove 'road closed' signs, even when the road is actually clear. 

There's no coordination between them but the traffic management is generally done by a third party. The doogooders insist that everything is in place before work starts and is cleared after the work is completed.
Ideally this is day before/after but delays and other job sites can mean a delay in work starting or traffic management being removed.

The days of crews bringing their own cones etc are sadly long gone.
