Out of line? It's an eccentric adjuter, would that explain it?

Obvious question is....How old is the belt? (Belt on a v6 omega means belt kit, belt, pulleys and tensioner to be replaced as one)

Schedule is 40k or 4 years.

If you are referring to the Aux Belt driven pulley then it's the waterpump :y

Ah sorry, thought you meant cam belt.

Yes water pump.

Omega A is Carlton. Nothing to do with omega apart from being part of the previous GM family.

Omega B is Omega pre face lift and face lift.

Long handled tool on 15 mm nut on tensioner pulley top left as look at the engine. Turn to the right/clockwise, then slip the belt off the water pump pulley in the centre. Then release tensioner to the left/anti-clockwise.

You'll need to remove a torx bolt on a clamp on one of the ac pipes to remove the belt fully if you need to.

But take a pic of the belt routing first for re fitting.

You need to remove the front cable tray and the aux belt tensioner...
