LPG paperwork / V5C

I need to send off the V5C to "add" the LPG conversion, if anyone has done this could they answer the following questions .....

1.  Section 7 - item 27 "Type of Fuel"  do I put "PETROL/LPG" or just LPG ??

2. The Guidance Notes say you have to provide "written details from the garage that carried out the conversion" ... is this a copy of the receipt or a copy of the LPGA Certificate ?? and will copies do or do they need the originals ??

Apologies for thread jumping Entwood --- but do I take it from question 2, that, if the V5 shows "petrol/LPG", the conversion was either LPGA installed or LPGA certified?

Reason I ask is that my beastie has, some time ago, been converted and the V5 shows "Petrol/LPG". My insurers want to know if it was a DIY job or LPGA. I have no certificate --- so ..... does the fact that it is registered and LPG mean it was a professional install ?

previous owner could have sent actual cert off...?

ENTWOOD I filled in the section saying "LPG/AUTOGAS" and i sent a copy of the certificate scanned on my computer, i even wrote on the bottom "photocopy"

they accepted that no problem and changed accordingly
