i have a elderly relative that i give a ride to quite a lot but she has a proplem wearing a seat belt she has some problems that if she has pressure on her side/shoulder she gets a lot of pain/brusing.now shes asked her doc for a seatbelt certificate but he wont give her one as its not bad enough.the way round it at the moment is she uses the lap belt in the rear but it takes so long to get her into the middle iam looking for another solution.is it legal just to fit a lap belt to the front seat?

I don't think you would be able to do that under the MOT laws and the fact that your car is fitted with the lawful seatbelts then they would have to be used to comply with the law. Options may be some king of padding on the belt where it rests on your relative and causes the problem.

As a general rule you have to use the seat belts fitted, with the obvious exception to the rule being a classic car without belts, you can then fit what you want 

A lap belt wouldn't suffice for the front unless the car had no belts in the first place

Is it both shoulders, or just the one?
There are pads that spread the load of the seatbelt so they don't apply too much pressure, but still remain effective in the event of an accident.
A friends grandparent uses them, her condition was severe enough to have a seatbelt exemption, but she felt it was unwise to travel without the use of a seatbelt, and now uses the pads and has suffered no pain since. I'll enquire as to where she got them from.

Check out seatbelt solutions for use in pregnancy, bump belt and similar, may help..may not.

.... but seriously, yes you're right in thinking that it is ILLEGAL to remove a 3-point belt and repalce it with a two-point belt on any outboard seat in a car made after 1981
